Perhaps she still really hates Reggie Lucas! And the first leg of the S&S Tour in 2008, lol. The only singles from "Madonna" she hasn't performed on tour for more than 10 years are Everybody (last tour performance at the Girlie Show if you don't count the Hung Up Promo Tour) and Physical Attraction (never performed on tour!). I'm praying we get to hear one or both on the next tour. It's time for them to return or appear on tour for the first time in the case of PA. Amsterdam. I bet it was rehearsed for MDNA. I remember a lot of speculation before the setlist was revealed saying she would sing it and then all of a a sudden she starts singing it at soundcheck.
