OMFG, NSFW warning please for the porn pic!!! Some of us innocently browse the new album thread from work/school or elsewhere in public. Save it for the smut threads in the lounge, or at least use the spoiler forum function.

Like A Prayer - 28
Vogue - 36 -
Most - GMAYL, surprisingly. At first I hated the song so much that I refused to even listen to it, but I've warmed up to it. Whenever I think of it I'm reminded of the amazing Super Bowl show and MDNA tour performance with the suspended drummers. The new Nicki Minaj rap/dance breakdown on the tour version also really improves it and should have been part of the original version of the song.
Least - Superstar. It's way too saccharine and sickeningly chipper and gets on my nerves just as much, if not more, than when it first came out. It reminds me of the other M song I absolutely can't stand - Cherish - in its same over the top cheeriness and annoying repetitiveness.

WTF is going on with the creepy-looking dude in his underwear handing out popcorn and the blimp on Madge's head? Is there any context?
-
Was this ever explained?
Probably a blepharoplasty aka eyelid surgery. That's another extremely common plastic surgery as people age because the skin there is so delicate and sensitive that it usually shows aging first. It's a big part of why M's eyes still look so fresh and beautiful today.

Like others have been saying, plastic surgery doesn't always look awful and terribly obvious. Most women in the public eye above the age of 45 who still look good have had some pretty serious plastic surgery (most often facelifts). Living healthily is definitely the most important part of aging well, but it doesn't keep jawlines taut or skin perfectly smooth. If Madonna still maintained the same diet and exercise routine but had never had surgery, she would have still looked younger than her age and like a very beautiful older woman but nowhere near as young and conventionally attractive as she appears now.
Other famous women have the benefit of not appearing in public as often or being as closely scrutinized as Madonna is so very good, natural-looking work can pass undetected. For example, Meryl Streep, Susan Sarandon, Julianne Moore, and Demi Moore look great but have definitely had work done. Diane Sawyer is 68 (!) but doesn't look out of place among younger news anchors. 56 year-old Sharon Stone, probably the most beautiful woman of her generation, still looks stunning compared to many 25 year-old starlets. Trust me, these women don't look this good still naturally.
Also, it's important to note that plastic surgery isn't just for purely cosmetic reasons - it was originally created to help people who have been disfigured like injured soldiers and accident survivors be able to live normal lives. For example, Lauren Hutton was in a horrific motorcycle accident in 2000 and suffered severe injuries but looks beautiful and normal today thanks to incredible plastic surgery.

I would also love for Harvard Business School to do a case study for Madonna - they've done one for Oprah Winfrey and other incredible successful entertainers as well.
There's an interesting case study from ESMT (the European School of Management and Technology) called Madonna - Strategy on the Dance Floor by Jamie Anderson and Martin Krupp. It's interesting how they break down the history of her career into various lessons business students can learn. CULTURALLY ENRICHING, as HolidayGuy would say

I love that they refer to Madonna's imitators as 'Madonnitas'. That should be the official title for any flop princess wannabes.

She's probably just in Michigan to visit with her family and added in a charity visit or two. The picture of her father and her kids at the beach was likely taken at a beach somewhere on Lake Michigan. The Great Lakes have some surprisingly stunning, grand beaches.

Discuss Madonna's promotional strategy (or lack of) here.

I definitely agree that it would be awesome for her to tackle the ageism thing head on! At this point, since very little is really controversial or shocking anymore, I think what it would be great for Madonna to do is to really create work with meaning. It seems like she's really gone in this direction recently with Art For Freedom and speaking out about censorship and political repression like with Pussy Riot in Russia and Marine Le Pen in France. Speaking about this in her work would be a great way to attract attention and remind people that she is still a very relevant recording artist with an important message. I remember during one of the MDNA promo interviews she said that she was still working so hard not because she has anything left to prove but because she had so much more left to say, and I think that's really going to be present in this upcoming era.
